MomCare Program

The MomCare Program will serve nearly 3000 women in Hardee, Highlands and Polk Counties this year.
What is the MomCare Program?

The MomCare Program is a program of the Healthy Start Coalition of Hardee, Highlands, and Polk Counties, Inc. It is sponsored by the Florida Department of Health and Florida's Agency for Health Care Administration.

MomCare provides information, referral and assistance for pregnant women covered by Medicaid.


Who is eligible for the MomCare Program?

Pregnant women with household incomes up to 185% of the federal poverty guidelines may be eligible for Medicaid insurance while they are pregnant.

How does a woman apply for the MomCare Program?

Applying for the MomCare Program and insurance is easy.

Have your health care provider document proof of your positive pregnancy test and expected due date on their office letterhead. Send that with a completed "Health Insurance for Pregnant Women" application to the address listed on the application. These applications are available at all local providers, including health department clinics.

Fill out applicant information in full. Do not omit any information or skip any questions or approval of the application will be delayed or denied. Make sure you sign the application.
