It is a short questionnaire completed in the doctor's office or the hospital. This determines risk factors that may possibly affect the pregnancy and the health of the baby. Answers are strictly confidential.
| Who Qualifies? | ![]() |
A doctor, midwife, or nurse will review answers to the screening form offered to you at your first prenatal visit to find out if extra help is needed during the pregnancy. The program is voluntary and available to pregnant women and young children up to age three. Eligibility is based on a screening score, not on financial factors. There are no fees for services. If you were not offered a screen by your doctor, please ask for one on your next visit.
